The course is renowned for its breathtaking scenery and challenging, yet rewarding, terrain. The first three miles wind through the Crazy Horse Memorial grounds on a mix of paved and gravel roads, offering rolling hills and a unique loop around the base of the mountain carving. Runners then transition onto the historic George S. Mickelson Trail, a crushed limestone and gravel path that meanders through golden birch trees, lush forests, open meadows, and past striking rock formations. While the marathon features a net elevation loss, it includes significant ascents and descents, with an out-and-back section to Custer before a final downhill push into Hill City. Half marathoners will enjoy a predominantly downhill and flat final ten miles to the finish.

Aid stations are strategically placed every 2-3 miles, well-stocked with water, Powerade, fruit, and pretzels, with a GU Gel station for marathoners.
